Financial Policy

Rebalance Wellness is a private wellness practice is a cash-based practice that does not accept/file insurance. If needed, we can provide you with a detailed receipt at the time of your visit that you can submit to your insurance provider for reimbursement. 

All payments are due at the time of service. We accept cash, credit/debit cards and HSA/FSA cards. You will need to check with your insurance company for HSA/FSA  coverage.  

To respect our time and that of other patients, we require at least 24 hour notice for any cancellations or rescheduling. Missed appointments or late cancellations will incur a  cancellation fee.

Bio-Identical Hormone Consultation Protocol

Treatment is indicated for: Hot flashes, night sweats, fatigue, weight gain, low libido, poor memory or concentration, loss of muscle mass, poor sleep, dry skin, hair loss, brittle fingernails, joint pain as well as many other symptoms. 

Laboratory testing: A full blood panel will need to be done before your first visit. This set of lab work needs to be fasting. We will go over this lab work at length during your initial visit. Lab work will be done in person at our clinic for your convenience and typically takes 7-10 business days to return. It will need to be resulted prior to your initial visit. If you have previously had bloodwork with your PCP, you can send us those results to avoid having duplicate tests done.

Initial BHRT Consultation: This new patient consultation is approximately 60 minutes. It includes a detailed review of symptoms and medical history, a review of laboratory results and a customized BHRT treatment plan.

Second BHRT Consultation: We require repeat lab testing 4-6 weeks after beginning the BHRT program. A second lab review consultation will then be scheduled to review symptoms and response to the BHRT protocol at that time. This visit is approximately 30 minutes in length.

Follow-up visits for BHRT: After your second visit, we require follow-up consultations with labs every 3-6 months. Visit frequency will be based on each individual and response to treatment.
